Provider Enrollment: Start Enrollment

The Provider Portal Start Enrollment page allows providers easier navigation when enrolling, revalidating, and re-enrolling their provider applications in a timely efficient manner.

1. Gather the required information listed on the Start page.
2. Click Continue or press the Enter key to begin the enrollment application.
3. Click Cancel to return to the Provider Enrollment page.
