Search Provider

Search criteria allows you search for a provider using distance (how far away the provider is from a particular ZIP code), the type of provider (physician, hospital, or pharmacy), and the provider's specialty.

Note: Two types of searches are available - distance-based and non distance-based. The configuration of your system determines which type of search you are presented.

Search for Providers

1.	Enter your address. You must enter either City and State, or Zip Code.
	Note: Address fields are case sensitive. If you live in Smithville, for example, you must enter it with a capital S or your search will fail.
2.	Enter the provider criteria. This includes the type of provider you are searching for, such as a hospital or doctor. You may also select a Specialty.
3.	Select a number of results to display per page
4.	Click Search Provider to return the results that match your search criteria.

You will receive a message if no providers meet your search criteria. Otherwise, your matching results appear.
